Public information about appointments, cancellations, payment and consent.
Appointments
Appointments are made through the Function & Forge booking system. Please arrive at the time stated in your booking confirmation.
Cancellation and rescheduling
I understand and appreciate that plans change. If you need to cancel or reschedule, please give at least 48 hours' notice. Cancellations with less than 48 hours' notice may be non refundable. Exceptional circumstances will always be considered.
Payment
Where payment is required at the time of booking, this will be shown clearly during the booking process.
Consent
Treatment will be discussed with you and requires your consent. You can ask questions, decline a particular treatment or withdraw consent at any point.
Scope and referral
Function & Forge provides treatment within the scope of sports therapy practice.
